This repository holds the forms and documents that you might need. If you paid your dues and joined, please download the DIA membership form, fill it out and mail it back to us. We are still working out the details of automating our online forms.
The documents on this page are all in PDF format so that people with low vision and vision impairments can more easily access them. Please let us know how we can make our documents and this site more accessible.
